Elden Ring's character customization offers a wide variety of ways to modify one's appearance, yet some players might not know that the game features unique eye alterations that occur once a specific action has been fulfilled. Similar to how the Dragonborn's eyes in the Elder Scrolls V: Skyrim changes once they've contracted Vampirism, the eyes of the Tarnished also alter in appearance after the protagonist has experienced a particular event or progressed through a certain questline.

Currently, there are three known eye alterations in Elden Ring that Tarnished can acquire: Dragon Eyes, Frenzied Flame Eyes, and Blood Eyes. As one might expect, Dragon Eyes give the player eyes like a Dragon, with a golden yellow iris/sclera and snake-like slits for pupils. On the other hand, Frenzied Flame Eyes turn the one's sclera ink-black while changing the iris and pupils into a maddening haze with colors resembling Frenzied Flame Incantations. Finally, Blood Eyes cause the eyes of Tarnished to become bloodshot, turning the outer eye red but leaving the pupils black.

The most straightforward eye alteration in Elden Ring is Dragon Eyes, obtained by purchasing four Dragon Communion Incantations using Dragon Hearts. These exchanges can be made at the Cathedral of Dragon Communion in Caelid or the Church of Dragon Communion in Limgrave.

To get the Frenzied Flame Eyes in Elden Ring, Tarnished must open the secret door at the bottom of the spire of corpses in the Subterranean Shunning-Grounds beneath Leyndell. To access this area, Tarnished must make their way through the underground dungeon sewers to reach the Cathedral of the Forsaken. After defeating Mohg the Omen, they can enter the hidden Illusory passage behind the site's altar and complete a tricky platforming challenge to reach the lowest level. Keep in mind that this location is accessible before and after the world-changing event in which Leyndell becomes the Ashen Capital.

Finally, the red Blood Eyes are acquired via progressing through White Mask Varré's questline. Also known as White-Faced Varré, this NPC is a devout follower of the Lord of Blood and wishes to invite Tarnished into his "bloodthirsty" family. During his questline, adventurers must retrieve the blood-soaked cloth of a Finger Maiden and invade other players' worlds three times. Once these prerequisites have been completed, Varré will proceed to tear the finger off of the protagonist and hand it back to them. Accordingly, the player's eyes will change to Blood Eyes once they've received the Bloody Finger from Varré.
